Thanks for your interest in Cousin Anne's Dog Academy.

To apply for entrance into Cousin Anne's Dog Academy, you will need to follow these three steps:

STEP 1: Contact Cousin Anne via form, email or phone call
STEP 2: SIGN UP to create your online account. Do this after we chat and are in agreement about moving forward together,
STEP 3: Login and enroll to Meet with Cousin Anne for your required in-person evaluation and/or phone consultation

Once your online account has been submitted and processed, you will be able to LOG IN to schedule appointments

Evaluation session requires all members of the family to participate

This session must be Pre-Paid in full.

The last 20 minutes I will take your dog for a walk so as to spend some one-on-one time with dog.During this time, I will want you to fill out the evaluation questionnaire that will help me to create your individual holistic lifestyle plan.

• I will evaluate the interaction between you & your dog

• Discuss what you want from your dog
• Discuss custom schedule of training

• I will evaluate your dog

• From this session, I will create our session plan

REQUIRED VACCINATIONS

6 — 8 weeks Distemper, parvovirus, Bordetella

10 — 12 weeks DHPP (vaccines for Distemper, adenovirus [Hepatitis], Parainfluenza, and Parvovirus) Influenza, Leptospirosis, Bordetella. Lyme disease per lifestyle as recommended by veterinarian. I highly recommend the Lyme disease vaccination.

16 — 18 weeks DHPP, Rabies, Influenza, Lyme disease, Leptospirosis, Bordetella

12 — 16 months DHPP, rabies Coronavirus, Leptospirosis, Bordetella. Lyme disease per lifestyle

Every 1 — 2 years DHPP Influenza, Coronavirus, Leptospirosis, Bordetella. Lyme disease per lifestyle

Every 1 — 3 years Rabies (as required by law)
